Practice Boundary Change : We Need Your Views

Our practice would like to apply to NHS Integrated Care Board who commission health care for local people to change the current boundary of the practice. We are doing this to make sure we provide quality services to our patients.

If the proposal is approved, it will mean that the practice is not able to register any new patients who live in the Ossett area unless they are a new dependant (new baby, adopted, or other dependant) of an existing patient.  If you are already registered with us and live within the Ossett area there will be no change for you unless you change address in the future.

For the extended area around City Fields, this will allow us to offer Primary care Services to this recently developed area. There is not currently one GP practice that covers the whole of the City Fields Development.

What is a practice boundary?

The local area a GP practice covers is called a boundary.  Sometimes a practice may decrease or increase a boundary to manage patient numbers. 

We need your views.

The practice would like your views on the proposal.  Your views will be considered by the practice and NHS Integrated Care Board to understand if the proposal will have any impacts that we hadn’t thought about.

To change their boundary, a practice needs to apply for permission to the NHS West Yorkshire Integrated Care Board (ICB), who plan and buy (commission) health care for local people, to change the current boundary of the practice.
 
The ICB will need to think about:
– Other GP services available
– The number of patients registered with the practice, the staff and building.
– The views of patients and stakeholders
– The impact any changes would have on patients
You don’t need to take any action if you are already registered with our surgery. You can continue to stay registered unless your address changes significantly and falls outside of our boundary area in the future.
We will actively plan for the increase in patient numbers by expanding our staff and resources to accommodate the additional patients. We are committed to maintaining and even improving the availability of appointments for both our current patients and those who newly register with us.

Our GP practice will soon be undergoing some refurbishment work. This will include the addition of 13 new consulting rooms as well as improving the layout and condition of the existing space. These improvements will make sure that we have the space to welcome more patients while also creating a more comfortable and efficient environment for everyone.
Do not worry, if you’re already a patient at Trinity Medical Centre you will not be removed.
If you are already registered with us and your home address is within the closed boundary, you do not need to change surgeries. However, if you move outside of this new boundary area in the future, you will need to register with a GP surgery closer to your new address.
If you are already registered with our surgery and someone moves into your home, such as a new child or a partner, they can still register with our surgery, even if your address is within the closed boundary area. Just inform the surgery that they are living with you before registering them.
You will still be able to stay registered with TMC. You will only be required to move surgeries if you move outside our full boundary.
Prescription deliveries are not managed by the GP surgery and depend on your chosen pharmacy. However, changing our boundary should not impact your ability to receive your repeat prescriptions as usual.
We anticipate a gradual increase in new patients. Many households in the City Fields area are likely already registered with a GP surgery, and since the development is planned to occur over a 10-year period, we expect manageable growth. This gradual increase will allow us to respond appropriately by scaling up staff and resources as needed.
